1. Fill­ing out nec­es­sary forms for school­ing, camp­ing, and access to other com­mu­nity ser­vices such as WIC. Please allow 24 hours for pro­cess­ing such forms which may be picked up from the Center.

Billing and Fees

Bills are to be paid as they are incurred.  Co-payments are payable at check-in.

Prior to each visit, make sure that your insur­ance is active. Patients with inac­tive insur­ance will be respon­si­ble for bills incurred.

Reach Out & Read

As a vital ser­vice to the com­mu­nity, Val­ley Children’s Med­ical Cen­ter pro­motes early lit­er­acy through its par­tic­i­pa­tion in Reach Out and Read (ROR) pro­gram. It pro­vides books to chil­dren six months of age and beyond.
